The Authority oversaw a Police review into their failure to carry out a welfare check in good time. When Police attended over 24 hours after the call to Police was made, the person was found to be dead. While the review found it was likely the person had died before the request for a welfare check was made, it did find several deficiencies in the way the matter had been handled. These issues have all been addressed with the staff involved.
Police have also made recommendations about how such matters should be dealt with in future to ensure welfare checks are given the appropriate priority.
The Authority agreed with the outcome and the actions taken.
