Shortcomings identified with a Police investigation into a serious crash in Southern District

In August 2018 a vehicle struck a pedestrian resulting in serious injuries. A Police review of the crash investigation identified several failings. The Police notified the Authority and commenced an employment investigation into the actions of the crash investigator (CI).

The Authority oversaw the employment investigation, which found that a report into the crash was not submitted within the six-month time limit for laying of charges and the CI's evidential statement contained factual errors. The Authority agrees with the findings of the Police investigation and the employment outcome applied to the CI.
