The Authority oversaw a Police review of a self-harm incident where the detainee wrapped her clothing around her neck.
Police undertook an initial risk assessment that identified the detainee needed frequent monitoring. Following the incident, the detainee was reviewed by the mental health team and no changes to the level of care were deemed necessary. A plan was put in place for the release of the detainee from Police custody.
The Police identified they failed to record details of their review of the risk assessment after the self-harm incident within the Police system, there was non-compliance with a requirement to frequently monitor, and they did not provide the detainee with her medication. These issues were addressed with the staff involved.
The Authority is satisfied with the Police review of this incident and actions taken.
